A Special Event in Support of the Whaling Museum

Come and enjoy a memorable day of local beer tastings and exciting activities inspired by the sea and its daring sailors. Listen to live sea chanteys, participate in a scavenger hunt for adults, and attend a gallery talk on the daring whaling voyages. 

 

This registration form is for vendors supplying beer tastings and representatives to dispense tastings for the following event:

Waiver Statement:

Vendor hereby accepts the following listed conditions and limitations:

 

VENDOR AGREES:

Vendor agrees to supply beer tastings and representatives to dispense tastings for the event. Number of beers and quantity to supply is vendor’s choice. Most vendors bring 1 or 2 kegs. Vendor may send up to 3 representatives to assist at the event.

 

THE WHALING MUSEUM AGREES:

The Whaling Museum agrees to supply outdoor space under one large tent, ice, 72" banquet table, pitchers for pouring and tasting cups. The Museum will promote the event and include vendor logo on promotional materials and webpage.

 

REQUIRED DOCUMENTS

1. Submit a Certificate of Insurance with The Whaling Museum Society, Inc. listed as additional insured for the day by April 21st, 2025. Email the certificate to Cristina Ainslie - cainslie@cshwhalingmuseum.org

2. Submit a vendor application for Suffolk County by April 21st, 2025 Temporary Food Service Vendor Application and Permit. Click on the link to download the application and complete. As part of this application you must also submit proof of possession of Worker's Comp and Disability Insurance or an approved waiver (Form CE-200) if coverage is not provided. (This is noted on the application form). Submit directly to Suffolk County & send copy via email to cainslie@cshwhalingmuseum.org when complete. 

 

INSTALLATION AND TEAR DOWN.

  • Vendor shall set up at the museum between 12:00 - 2:00PMpm, on June 7, 2024 and shall remove his/her facilities from the Museum no later than 6:00 pm on June 7, 2025.

  • Vendor is responsible for staying the entire duration of the event and is responsible for all tools, supplies, set up, clean up and everything related to their vendor table.

 

PAYMENT. There is no payment exchange between the Vendor & Museum.

 

APPEARANCE. Vendor is responsible for cleaning and maintaining the space provided in an organized and neat manner. The Vendor will remove bulk trash at end of event.

 

EXTRA SERVICES. The Whaling Museum is not obliged to provide telephone, electrical or any other services to the vendor.

 

DISPLAYS AND SIGNS. All displays & signs must be free standing. Nothing may attach to walls or columns of the building by any means at all. Signs should not block other vendor's space or walking traffic. Signs may not attach to the walls or columns of the building at 279 Main St.

 

INSURANCE. The Whaling Museum requires to be listed as “Additional Insured” for the day of the event. In addition, The Whaling Museum accepts no liability for lost, stolen or damaged property and is not required to carry additional insurance to cover vendor’s property.

 

INDEMNIFICATION. The vendor agrees to indemnify and hold The Whaling Museum and Education Center harmless from all claims, losses, expenses, fees including attorney fees, costs, and judgments that may be asserted against the Museum that result from the acts or omissions of the vendor and/or The Whaling Museum’s employees, agents, or representatives. The vendor shall be solely responsible for ensuring all applicable laws are followed and complied with supplying their products and services at the Event.

 

COVID-19 INDEMNIFICATION. On January 30, 2020, the World Health Organization (WHO) designated the novel coronavirus, COVID-19 outbreak as a Public Health Emergency of International Concern. By agreeing to participate in the Museum’s event, all parties acknowledge there is an assumption of exposure to COVID-19 and any and all consequences and/or injury which may result from such exposure, including but not limited to, physical and/or psychological injury, pain, suffering, illness, temporary or permanent disability, death or economic loss. Vendor holds The Whaling Museum Society, Inc., and its trustees, employees, or other representatives, harmless for any injuries or any other loss associated with the Coronavirus that may occur as a result of participation.

 

PHOTO RELEASE. Vendor grants The Whaling Museum permission to use Vendor’s name, images submitted, branding materials, and any photographs or video taken at the June 7th event of vendor’s items and/or representatives for advertising and publicity purposes, both printed and digital, including social media, without payment or other consideration.

 

GENERAL RELEASE. I have read the rules as described above. My signature indicates that I make this agreement in good faith and am ready, willing and able to participate in the event. I understand that written notification of acceptance into the event implies a contract with all duties and obligations incumbent therein.

 

PERSONAL RESPONSIBILITY WITH ALCOHOL:

  • I agree that any alcoholic beverages I consume are my own choice and responsibility. I agree to drink responsibility and heed suggestions from others and will not drive if I am intoxicated. I acknowledge that I alone am responsible for all of my actions resulting from consuming alcohol and the Museum is in no way responsible.

  • I acknowledge that The Whaling Museum itself does not sell alcoholic beverages for events held on its property.

  • I agree to idemnify and hold hamless the Whaling Museum and their representatives, from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of my participation.
